Where to Get Care and Support for
Shwachman-Diamond Syndrome

The following facilities are medical centers that Shwachman-Diamond Syndrome  Foundation recommends for treatment of SDS. These are hospitals that have doctors who are knowledgeable of Shwachman-Diamond Syndrome and some of these doctors are even on our Medical Advisory Board. Each center will arrange appointments as needed for hematology, GI, endocrinology, orthopedics and more. These Shwachman-Diamond Syndrome experts will draw up a patient care plan for the patient to take home to their own physicians who routinely follow them.

The Shwachman-Diamond Syndrome Foundation maintains an SDS Physician Directory, which contains a directory of physicians who currently follow Shwachman-Diamond Syndrome patients. Physicians who are knowledgeable of SDS are oftentimes requested from the Shwachman-Diamond Syndrome community and we thank you for your participation to help us compile this list of physicians.

The information below will be asked of each SDS patient/family filling out the form, which may take you 10-15 minutes to complete.
